Two Glasses, One Day Gone: Alcohol After 40

The short answer: Alcohol affects people more with age for three separate reasons. Body water declines, so the same drink produces a higher blood alcohol concentration. The enzymes that clear it become less efficient. And alcohol disrupts the second half of the night, which is already becoming more fragile with age.

Two glasses of wine with dinner. Nothing remarkable, nothing late.


Then a Wednesday that feels like it has been dragged uphill. Foggy by mid-morning, flat by three, and disproportionate to what was actually consumed by a margin that seems almost unfair.


Most people file this under tolerance and assume they have simply gone soft.


That is not what is happening. Three separate things have changed, and only one of them has anything to do with tolerance.


The Same Drink, A Stronger Dose


Alcohol distributes through the water in the body, not the fat.


This is the part that quietly changes. As lean mass declines and the proportion of body fat rises, total body water falls as a share of body weight. Less water means less dilution, which means the same drink produces a higher blood alcohol concentration than it used to.


Researchers measure this as the volume of distribution, and it has been quantified by decade. A study of healthy men found it fell steadily across age groups: roughly 0.72 litres per kilogram in the twenties, 0.71 in the thirties, 0.68 in the forties, and 0.66 in the fifties.


Those numbers look small. They are not, because the effect is multiplied across every drink. The same two glasses that produced one blood alcohol level at thirty produce a measurably higher one at fifty, with no change in behaviour whatsoever.


The same study found something else worth noting: subjective feelings of intoxication correlated with age. Older participants did not just have more alcohol in the blood. They felt it more.


So the sense that drinks are hitting harder is not imagined, and it is not a failure of tolerance. It is arithmetic.


The Clearance Is Slower


The second change is on the way out rather than the way in.


Alcohol is broken down mainly by two enzymes, alcohol dehydrogenase and aldehyde dehydrogenase, with a third pathway involving cytochrome P450 2E1. The activity of these enzyme systems has been reported to diminish with advancing age.


Higher concentration on the way in. Slower clearance on the way out. The result is that alcohol lingers, and the window in which it is still circulating stretches further into the night than it once did.


Which brings us to the part almost nobody accounts for.


The Damage Happens While You Are Asleep


Alcohol is widely misread as a sleep aid, and there is a reason for the confusion. It genuinely does shorten the time taken to fall asleep, and in the first half of the night it increases slow-wave sleep, the deepest stage.


That is the whole basis of the nightcap.


But a meta-analysis of 27 studies by Ebrahim and colleagues, published in Alcoholism: Clinical and Experimental Research in 2013, described what happens next. The effect is biphasic. Sedation in the first half of the night, and then, as the alcohol is metabolised, a rebound: REM sleep suppressed, wakefulness increased, and the second half of the night substantially fragmented.


The night divides into two halves that behave in opposite directions. The first feels excellent. The second quietly takes it back.


And critically, none of this is dependent on drinking heavily. These effects have been observed at low to moderate doses, and are more pronounced the closer the drinking sits to bedtime. A recent systematic review and meta-analysis by Australian researchers, published in Sleep Medicine Reviews in 2025, examined the same question in healthy adults.


This is why the day after two glasses can feel so out of proportion. It is not really a hangover in the conventional sense. It is a night of sleep that was structurally worse than the total hours suggest.


Why This Compounds After 40


Here is the part that ties the three changes together, and it is the reason the effect grows rather than staying constant.


The second half of the night is already becoming more fragile with age, independent of alcohol. Slow-wave sleep declines steadily from young adulthood. Brief awakenings become more frequent. The body clock drifts earlier.


Alcohol targets exactly that portion of the night.


So the disruption is not being added to a robust system. It is being added to one already trending in the same direction, on top of a higher blood alcohol concentration that is clearing more slowly. Three effects, all pointing the same way, all arriving together somewhere in the forties.


That is why the change often feels sudden despite the underlying shifts being gradual. Nothing broke. Several small things simply stopped cancelling out.


The Difference Nobody Mentions


There is a sex difference here that is well established and rarely stated plainly.


Women have proportionally less body water than men at the same weight, which means the dilution effect described above starts from a lower baseline. There is also a difference in gastric alcohol dehydrogenase activity, which affects how much alcohol is broken down before it reaches the bloodstream.


The practical consequence is that a woman and a man of identical weight drinking identical amounts will not arrive at identical blood alcohol concentrations. The woman's will be higher.


Since body water declines further with age in both sexes, this is a gap that does not close.


What This Actually Means


Not that anyone needs to stop. This is an explanation, not a prescription.


But it does reframe a common assumption. The instinct is to treat the changed response as a loss of tolerance, something that might be trained back with practice. The mechanisms above suggest otherwise. Higher concentration, slower clearance and a more fragile second half of the night are structural, not behavioural.


Two things follow, and both are just physiology rather than advice.

The timing matters more than it used to. Alcohol closer to bedtime overlaps more completely with sleep, because clearance is slower than it was.


And the cost is showing up in a different currency. Much of what feels like a hangover after a modest amount is closer to a sleep debt, which is why it presents as fog and flatness rather than headache and nausea.


Australia's national drinking guidelines are published by the NHMRC and are worth reading rather than assuming. Anyone with genuine concerns about their drinking is best served by a conversation with their GP, who can look at the full picture properly.


The two glasses have not changed.
